Reports & Evaluations
Mental health reports completed on a case by case basis (no custody cases). Reports may include requests made by employers, immigration, and others third party requestors.
Reports created for the purposes of an evaluation are not therapeutic in nature and aim to produce objective findings that can be substantiated based on information gathered, and "may include examination of the individual and/or review of records" (ACA Code of Ethics, E.13.a, 2014). Considering the nature of the evaluative reports, they must be requested and agreed upon in writing at the time of an initial appointment.
Request for reports by established clients/supervisees will be limited to the nature of the counselor-client or supervisor-supervisee relationship. Reports beyond the standard practice will need to be agreed upon in writing. Reports beyond the scope of practice may result in a referral to another professional.
